How to Obtain Medical Records

The following instructions are for health information management services at Cincinnati Children’s Hospital Medical Center.

Forms may be submitted in person, by mail or fax.

CASA Volunteers are asked to obtain all medical records for the child(ren) they are serving. The CASA Volunteer’s Court Entry Letter must be included whenever requesting records. This letter is the court’s authorization allowing the CASA Volunteer to obtain all records concerning the child while in the custody of Hamilton County Job and Family Services. The medical information should be included in the Court Report. To obtain information of the biological parent or any other party to the case, a Release of Information form (below) must be signed by the party.
